How Over 1,000 Therapists Successfully Scaled to Group Practice: Real Results and Lessons Learned

Welcome therapists and group practice owners! I’m Nicole McCance, a psychologist turned business coach for therapists. I scaled my own practice to 55 therapists and multiple seven figures before selling it, and now I help other therapists do the same thing. 

After coaching over 1,000 therapists (yes, really!) through the process of building and scaling group practices, I’ve noticed some clear patterns. There are specific challenges that come up again and again, and there are also specific things that make the difference between a practice that grows sustainably and one that leaves you overwhelmed and burnt out.

Here’s what I’ve seen make the biggest difference for therapists who successfully scale to group practice.

The Overwhelm of Not Knowing Where to Start

One of the most common things I hear from therapists who want to scale is that they feel completely overwhelmed. You know you want to grow, but you have no idea where to start or what to prioritize. Should you hire first? Get your paperwork in order? Figure out your marketing? The questions pile up, and it’s easy to get stuck before you even begin.

What Therapists Tell Me About Feeling Stuck

The overwhelm typically shows up in a few ways:

  • Not knowing what order to tackle things in
  • Feeling paralyzed by all the decisions that need to be made
  • Worrying about making expensive mistakes
  • Questioning whether you’re even ready to scale

What I’ve found is that therapists don’t need more information. There’s plenty of advice out there. Here’s something I wish I had known earlier in my own journey (because trust me, I made some serious mistakes): every mistake you make while scaling costs you time, money, or both. 

Whether it is hiring the wrong person, setting up your legal structure incorrectly, or making HR decisions that come back to bite you, these mistakes add up fast.

Common Mistakes That Cost Practice Owners

The most expensive mistakes I see therapists make include:

  • Misclassifying employees as contractors
  • Failing to put the right systems in place so their practice can run without them
  • Making hiring decisions without proper vetting processes
  • Setting up compensation structures for their team that are unsustainable
  • Skipping foundational steps and trying to scale too quickly

The therapists I work with who are most successful are the ones who learn from other people’s mistakes instead of having to make all of them themselves.

I built my program specifically to help therapists avoid the mistakes I made and the mistakes I’ve seen hundreds of other practice owners make. 

When you have access to vetted resources, like HR consultants, employment lawyers, and accountants who understand the therapy world, you’re not learning through trial and error. You’re building your practice on a solid foundation from day one.

It’s lonely, and that loneliness makes it harder to keep going when things get tough.

Why Community Matters More Than You Think

What I have seen is that having a community of people who are building the same thing you are building makes a world of difference. Here’s what community provides:

  • Validation that your challenges are normal
  • Real-time problem-solving from people in the trenches
  • Celebration of wins with people who truly understand
  • Accountability to keep moving forward

When you are surrounded by other therapists who are also scaling, you realize that the challenges you are facing are normal. You get to celebrate wins together, troubleshoot problems together, and learn from each other’s experiences.

After working with over 1,000 therapists, here’s what I’ve learned: scaling successfully isn’t about having all the answers. It is about having the right support, systems, and mindset.
